ABSTRACT

Ocular histiocytosis is a rare and heterogeneous group of disorders which can occur in children and adults. There is a great challenge in the diagnosis and treatment because of the atypical clinical and imaging manifestations. With insights into molecular mechanism of histiocytosis, the discovery of BRAFV600E mutations has changed the understanding of this disease and enabled targeted therapies in most patients. The gold standard of diagnosis has developed into histopathological biopsy combined with the testing for mutations. Surgery is not the only treatment for ocular histiocytosis and targeted therapy has become an effective treatment for patients with mutations in MAPK-ERK signal-regulated kinase pathway. However, the greatest challenge for ocular histiocytosis is establishing the early and correct diagnosis due to the diverse types and clinical manifestations. Therefore, this article reviews recent progress in diagnosing and treating ocular histiocytosis, summarizes their clinical and pathological features, and aims to improve the level of diagnosis and treatment among clinicians.

ABSTRACT

BackgroundOcular melanoma is a malignant tumor with a poor prognosis.To study its mode of pathogenesis and development is helpful for early diagnosis and treatment and improving prognosis. Objective This study was to analyze the location and pathological types of ocular melanoma,and explore the relationship of pathological types with prognosis of ocular malignant melanoma. Methods 110 clinical data and specimens of ocular melanoma were collected at West China Hospital,China.The specimens were examined by hematoxylin & eosin staining,immunochemistry and studying their ultrastructure.The growth location,histological classification,and their relationship with prognosis were analyzed and discussed.The clinicopathological records of the 1 l0 cases with ocular region malignant melanoma treated from January 1980 to December 2007 were analyzed.Results The primary locations of the tumors with respect to the tissue type were:66 cases in the uvea (60.00% ),27 cases in the ocular surface tissue (24.55%),8 cases in the secondary lesion of orbit cavity (7.27%),7 cases on the eyelid skin (6.36%),and 2 cases in the lacrimal sac ( 1.82% ).The neoplasm was classified according to pathological types:spindle-cell A type in 31 cases (28.18% ),spindle-cell B type in 31 cases (28.18%),mixed-cell type in 25 cases (22.73%),epithelioid-cell type in 17 cases ( 15.46% ) and other type in 6 cases (5.45%).The notability of diffusion or recurrence rate of the 5 pathological types showed statistical significance (x2 =14.463,P =0.006 ),and the rank in decending order was:other type,epithelioid-cell type,mixed-cell type,spindle-cell B type and spindle-cell A type.The clinical manifestations of ocular melanoma were dependent upon the lesion position without specificity.ConclusionsThe majority of the ocular malignant melanoma appears in the uvea and ocular surface tissue,and its common pathological types were spindle-cell type followed by mixed-cell type,epithelioid-cell type and other type.The highest recurrence rate is in other type of ocular malignant melanoma.Ophthalmologist should be aware of the clinical manifestation of ocular malignant melanoma.

ABSTRACT

· AIM: To report a rare case of mesenchymalchondrosarcoma in the orbit and to explore its clinicmanifestations, pathologic characters, management andprognosis. · METHODS: We report a case of mesenchymalchondrosarcoma of the orbit. The clinical materials,including ophthalmological examination, computed tomo-graphy scan of the orbit, histopathology and immunohis-tochemistry of the biopsy specimen was reported, and itspertinent literatures were reviewed.· RESULTS: A 36-year-old female was seen with proptosisand decreased vision. Histopathology demonstrated anadmixture of undifferentiated mesenchymal cells andislands of mature hyaline cartilage. Immunohistochemicalstudies revealed positivity for vimentin and S-100, whichwas consistent with the diagnosis of mesenchymalchondrosarcoma.· CONCLUSION: Mesenchymal chondrosarcoma in theorbit is extremely rare malignant tumor. Multi-modalitytreatments (surgery, chemotherapy and radiotherapy)may lead to long-term survival.

ABSTRACT

·Diffusely infiltrating retinoblastoma is a rare histopatho-logic type of retinoblastoma, accounting for approximate-ly 1% to 2% of retinoblastoma cases. We report a case of diffuse infiltrating retinoblastoma presenting as endo-phthalmitis and comment on the histopathologic findings.

ABSTRACT

<p><b>AIM</b>To study the influence of enterococci on human sperm membrane in vitro.</p><p><b>METHODS</b>Ejaculated human sperm were artificially infected with beta-hemolytic or non-beta-hemolytic enterococci at the bacteria: sperm ratio of 50:1 at 37 degrees . Sperm membrane integrity was examined after incubation for 1, 3 and 5 h by hypoosmotic swelling (HOS) test and electron microscopy.</p><p><b>RESULTS</b>Sperm infected with beta-hemolytic enterococci had lower HOS scores compared with non-beta-hemolytic strains or uninfected control (P < 0.01). The HOS test scores of sperm infected with beta-hemolytic enterococci increased in the presence of phosphatidylcholine, an inhibitor of hemolysin. Non-beta-hemolytic strains showed no significant difference in swelling rate, compared with the control group (P > 0.05). It was shown by electron microscopy that beta-hemolytic enterococci caused significant rupture of human sperm membrane.</p><p><b>CONCLUSION</b>Beta-hemolytic enterococci caused human sperm membrane injury, and might be mediated by the hemolysin of enterococci.</p>

ABSTRACT

<p><b>OBJECTIVE</b>This study aimed to compare the prevalence of mental disorders and services for people with mental health problems in Shaoxing city between 1991 and 2001 in order to provide scientific bases for prevention and treatment.</p><p><b>METHODS</b>A population size of 339 651 were investigated in the urban and rural areas with "Psychosis Index Investigate Schedule" plus visit to their homes.</p><p><b>RESULTS</b>The overall prevalence rates of mental disorders were 10.17 per thousand and 13.85 per thousand and the point prevalence rates of mental disorder were 9.15 per thousand and 12.77 per thousand, respectively in 1991 and 2001. The point prevalence rates in the population aged 15 years and above were 11.70 per thousand and 14.60 per thousand. Prevalence rates of neurosis and alcohol dependence and drug dependence increased significantly from 1991 and 2001. The anterior three mental disorders were neurosis, schizophrenia and dementia in all mental disorders. Situation was better in the city than in the rural areas in terms of receiving care for mental disorder, their work ability, economy condition and gardianship.</p><p><b>CONCLUSION</b>There was an increase of prevalence on mental disorder in Shaoxing city during the ten-year period. The increase in the prevalence of mental disorder was related to local economy, socialization and culture.</p>
